Let me adjust my tinfoil hat here and say this:

He is a Trump admin asset who went to see the conditions at the El Salvador concentration camp, where screwworms are common amongst the prisoners and some guards.

Only difference is guards likely get treated for it, and are told not to talk about.

Our Maryland patient was not told about the worms eating people as a convenient and plausibly denialable execution method, or that he might catch it in their facility.
